The Scheduler / Support Assistant is primarily responsible for scheduling physicians and nurse practitioner visits for hospice and palliative care. In addition, the Scheduler / Support Assistant will provide a wide variety of clerical duties for the Elizabeth Hospice and the Elizabeth Supportive Medical Specialists.
JOB DUTIES/KNOWLEDGE/JOB PERFORMANCE:
Responsible for scheduling patient visits for providers in the hospice and medical group EMRs and creating Patient Tasks upon request from incoming calls and/or emails from Team Managers, RN Case Managers (RNCMs), Admission staff, LVNs, and MSWs. May include coordinating patient visits with Team Managers, RNCMs, LVNs and MSWs as requested.
Responsible for coordinating urgent visit requests per the direction of the Medical Group Practice Manager, Chief Medical Officer, or designee.
Responsible for scheduling Face-to-Face (F2F) visits upon admission as requested from the Access Center.
Responsible for running the F2F Report in a manner approved by the manager or designee and scheduling F2F visits.
Responsible for scheduling patients, who reside in SNFs, for a monthly visit by a provider.
Responsible for confirming patient visits by calling patients and/or responsible party, documenting calls/confirmation of visit in EMR and emailing providers, Team Manager, and RNCMs of provider visit.
Responsible for any scheduling changes, updating changes in EMR, and notifying provider and patient (if requested) of change.
Responsible for receiving incoming calls from provider, addressing needs of provider, and exercising judgment as to urgency, nature, and confidentiality of the call.
Responsible for reviewing unverified report and incomplete tasks list weekly to ensure physician documentation has been submitted in a timely manner.
Responsible for informing Medical Group Practice Manager and CMO of any incomplete documentation by provider.
Responsible for tasking the patient care coordinators to fax verified notes to the community physicians and facilities, and updating report to reflect completion.
High school diploma or equivalent.
Minimum experience of 2-3 years in clerical/office setting: healthcare field required.
Possess strong phone etiquette/customer service skills, including professionalism, active listening, and speech clarity.
Ability to handle complaints and difficult situations.
Demonstrates ability to communicate effectively with staff, management, patients, physicians and community agencies, both verbally and written.
Possess strong organizational skills, including attention to detail.
Ability to manage time efficiently.
