Under direct supervision, provides care coordination and case management services to families via phone, office, home, or community setting. These include: providing educational information to patients, parents and families; providing outreach and support services to families; serving as an advocate, skill coach, and role-model; developing support systems and linking families with community resources; and providing case consultation with multiple providers and team members. Serves as a liaison with community referral sources to facilitate referrals, service delivery, and treatment options. Conducts data entry per program requirements. Conducts community outreach as needed. Completes assignments by adapting with flexibility to necessary changes in schedule, case assignments, and procedures for changing work demands and continuity of the program and services.
Valid CA driver's license and auto insurance required (if applicable)
Computer skills sufficient to perform data entry, electronic charting, production of educational materials and word processing
Demonstrated ability to maintain and manage caseload, complete records in timely manner, to provide a variety of community services, and to work effectively and harmoniously with professional and clerical personnel.
One year of experience
Additional education beyond undergraduate degree related to area of specialty
Thorough knowledge of child development
Thorough knowledge of community resources for children age birth to five years
